Event documentation fails in predictable ways, usually at the joins between the risk docs and the operational plan. Most of those failures are invisible until something goes wrong. These are the three we see most often.

You inherited a risk register from someone who is no longer in the role. The hazards have controls listed. The matrix looks professional. The version control field has someone's name in it. Nobody has opened the file since it was signed off. The extracted text content is sent securely to an AI assessment service (hosted by Anthropic via Cloudflare) to produce the score. This text is processed in real time and discarded after the assessment completes. We never see or store your document.
